Issue:

Adding a Revision in the Revision History Table Generates a New Table Instead of a New Row in Fusion 360.

New revision table appears when adding a revision
 

Causes:

 This is feature as designed to split the table.

Solution:

To solve this issue in Fuison 360 follow the steps below:
  1. Select the Revision History table by Right-Clicking on it.
  2. Drag the Blue arrow to make the table bigger.
  3. Now the earlier added rows appear on the list.
